Centre-Based Individual Care
At Sunland Support, Centre-based Individual Care provides one-to-one disability support worker assistance that helps participants join everyday, social and community activities. Every participant has their own dedicated support worker, including during group activities.
A centre or activity is the setting for disability support. You can use eligible NDIS Core funding to pay for agreed disability support worker assistance. We confirm the relevant support item against the participant's plan and the assistance delivered. Ordinary admission, food and recreational costs are not automatically included.

Disability Support Available
Disability Support Available After School
Sunland Support provides individualised disability support worker assistance after school, including daily living, personal care and social participation based on the participant's goals. We confirm support arrangements and available sessions individually. Cromehurst Cottage is operational for current Cromehurst School students; general Centre-based Individual Care has separate eligibility and delivery arrangements.
Disability Support Available on Weekends
Sunland Support's disability support workers provide agreed weekend disability assistance with social participation, everyday skills and community access. We confirm staffing, times and locations individually. You can use eligible NDIS Core funding for this worker assistance when it matches your plan and the support delivered.
Disability Support During School Holidays
During school holidays, Sunland Support provides individualised disability support worker assistance, including daily living, personal care, social participation and community access. We confirm your child's session and staffing arrangements before support begins. Ordinary activities, meals and entry charges are separate expenses unless specifically funded.
Learning & Leisure
Art & Creative Workshops
Our disability support workers assist participants with agreed creative activities suited to their interests and abilities. Assistance can include communication, choices, everyday participation and safe engagement with materials. Creative activities are not automatically clinical art therapy.
Supported Community Outings
Our disability support workers assist participants with agreed visits to local parks, museums and other community places, taking account of each person's needs and goals. Eligible NDIS Core funding pays for qualifying worker assistance. We explain tickets, meals, transport and ordinary activity expenses separately before the outing.
Sports & Recreation
Suitable movement, games and recreation can create opportunities for social participation and everyday skills. Support workers can provide disability-related assistance as agreed for the individual, considering access, safety and the participant's preferences. Recreational activity costs are not automatically NDIS-funded.

Cromehurst Cottage: Disability Support in Lindfield
Sunland Support provides centre-based disability support at Cromehurst Cottage in Lindfield for children and young people currently enrolled at Cromehurst School. Our experienced disability support workers assist with daily living, personal care, social participation and community access.

One child. One dedicated support worker. Every child has their own one-to-one disability support worker, including during individual and group activities. Children participating together does not mean their workers are shared.

A familiar playground, directly connected
A gate leads directly from the Cottage's backyard into Cromehurst School's securely fenced primary playground, already familiar to the children. Each child has their own disability support worker during suitable supported activities.
Your child's specialists are welcome
Your child's existing occupational therapist, speech pathologist, dietitian or other treating specialist is welcome to visit by arrangement and coordinate with Sunland Support with your permission. Their professional services are separately arranged.

Yes. You can use eligible NDIS Core funding to pay for agreed disability support worker assistance. We confirm the correct support items, rates and individual plan arrangements before services begin. Ordinary meals, admission tickets and recreational expenses are not automatically funded.
